I would include the source always. Crunchbase, wikipedia is always a good source but be aware. Slideshare is a great tool for this. Always cite source.

Michael is right, "Venture Deals" (second edition) by Brad Feld is by far the best book and includes a sample term sheet. It is less about angel investment but they do cover convertible notes. This is an alternative to the CN. Also, here is a link to some startup books.

If you haven't read Lean Startup, read that before pursuing any investment. Hold on and bootstrap as long as you can is my advice.
